This form is to be completed if you have already been a successful applicant under the previous CVIS Scheme or you have a contract and a vacant property agreement in place that you have or are about to occupy.

Checklist of Required Items

Please ensure that you have following information to hand BEFORE commencing your application;
 

1. Active street front evidence - photograph of front of premises
2. Derelict Site Registration Number (if relevant);
3. Dangerous Site Registration Number (if relevant);

These items will be required before grant approval once the initial application has been received. We will contact you directly to supply these items;
 

1. Bank statement header for business account (if year 1) 
2. Completed Supplier Set up form if applicable (if year 1) 
3. Customer Rates Reference Number (located on your Customer Statement);
4. Copy of Tax Certificate - online (https://www.ros.ie/myaccount-web/home.html or https://www.ros.ie/ros-registration-web/ros-registration
5. Evidence of Vacant & Offered for rent or lease for minimum 3 months prior to occupancy (can be obtain form your estate agent of landlord' you can also ask your Revenue Officer for a confirmation letter);
6. Evidence of Minimum 12 month Lease or Evidence of ownership (must be signed and dated, with clear information about the length of the agreement) as draft will not be accepted;
